Summary is plain text again (markdown showed as symbols); add a "View monster page" button that links to the monster

Monster dropdown: only show rows not hidden from the planner

Monster picker reads the Monsters dataset (planner_hidden = false, planner_label), and the summary links the monster with its level. Accept after the refreshed Monsters upload.

Gear planner: Monster dropdown uses the Gear Planner Monsters list
