The required level is the item level divided by 10, from 1 to 70. Ancient: 10% of its drops from level 70 enemies and chests and of Mystery items bought at player level 70 (9.98% where Black Mist comes first); twice the upgrade gold and sell price. Black Mist: 0.2% of those drops on Nightmare and Inferno and of those Mystery items once Nightmare is unlocked; requires level 70, never Ancient, tradable until its first reveal.

Legendary drops
Drops from enemies of level 50 and higher and from treasure chests they drop, only for heroes of its classes.
Mystery Belt
Sold for Soul Shards by Lady "Shadow", the Mystery Vendor in each town: one Belt of random rarity for the current hero. Online characters can buy it too, at the price the game server sets.

Before upgrades, by item level. An enemy of level E drops item level 10 x (E - 1) to 10 x E + 9 on Normal, 700 and higher on Nightmare and Inferno. Each number rolls on its own from 0.85 to 1.15 x its base. An Ancient drop takes the top of every range.

Attribute = 1.6 x item level x its value per item level x the roll. Ranges at item level 700 and 850:
